How do the tiers work?

Drug costs are broken down into 5 understandable tiers.  Take a look:

Tier 1

$10 Maximum Cost

Any drug that has a plan cost of up to $10.

Tier 2

$20 Maximum Cost

Any drug that has a plan cost over $10 but no more than $20.

Tier 3

$30 Maximum Cost

Any drug that has a plan cost over $20 but no more than $30.

Tier 4

$50 Maximum Cost

Any drug that has a plan cost over $30 but no more than $50.

Tier 5

Over $50 Maximum Cost

Any drug that has a maximum cost over $50.

Drug prices may vary from one location to another and are subject to change without notice. Prices shown are based on a 30 day supply. Additional quantities may incur higher costs. Discounts are available only at participating pharmacies. This program provides discounts at participating pharmacies for prescription drugs. This program does not make payments directly to participating pharmacies. The cardholder is responsible to pay 100% of the purchase price for all prescription drugs. The purchase price includes the discounted cost of the prescription drug plus all dispensing fees. The cardholder pays the lower of the discounted prescription drug cost plus dispensing fees, or the participating pharmacy’s cash price to customers.

These plans do not meet the Minimum Creditable Coverage requirements under M.G.L. C. 111M and 956 CMR 5.00. These plans do not qualify as a health plan under the Affordable Care Act. The Discount Medical Plans described in this brochure are provided by Patriot Health Florida, Inc., A Discount Medical Plan Organization. The plan is not a health insurance policy and is not available in all areas. The plan provides discounts to certain health care providers for medical service and does not make payments directly to the providers for medical services. The range of discounts for medical services provided under these plans will vary depending on the type of provider and service received.
